What is the Botox Lip Flip?

A lip flip is one of the smallest treatments we do, and one of the most asked about. It takes about ten minutes, uses a few units of Botox, and costs $125 flat at The Spa. There is no syringe of filler and no real change to the shape of your face. Just a little more of your upper lip on show when you talk and smile.

Here is the part people get wrong most often. A lip flip does not add volume. It will not make a thin lip bigger. What it does is relax the muscle along your upper lip so the edge rolls gently outward and shows more of the pink part that normally tucks under. The result reads as a fuller, softer top lip, but nothing was actually added. If you want size and structure, that is lip filler, which is a separate treatment that uses hyaluronic acid to build real volume.

The two get compared constantly, so we like to be plain about it. A lip flip is $125 and works with the lip you already have. Lip filler runs $550 to $925 per syringe and adds volume you did not have before. Filler also lasts far longer. A flip is subtle and short-lived by comparison. Plenty of people do both, and some start with a flip to feel what a little more upper lip is like before they commit to filler.

Whichever way you are leaning, the honest answer usually comes out of a quick look at how your lip moves. Our injectors will tell you which one actually fits what you are after, and they are fine saying "neither yet" if that is the real answer.

What to expect from your Lip Flip appointment

The appointment is quick. A few small injections along the top lip and you are done. You will not see much right away, though. A lip flip takes a few days to start showing and reaches its full effect in about one to two weeks, so do not judge it the same afternoon you leave.

It also does not last long, and that is worth knowing before you book. Results tend to run on the shorter side, often around 6 to 8 weeks, because it is a small amount of Botox working in a muscle you use all day long. That is much shorter than filler, which is part of why a flip works nicely as a first step or a low-commitment refresh.

For the first week or so your lip may feel a little different. Some people have trouble sipping through a straw, whistling, or forming sounds like P, B, and M, since those all need the lip to press together. A slight unevenness between the two sides can happen as well. These effects are temporary and ease off as the Botox wears down. Please tell us if you are pregnant, breastfeeding, or have a neuromuscular condition, because a lip flip is not appropriate in those cases.

A lip flip uses 4 to 6 units of Botox placed along the upper lip line. That is the entire treatment. The Botox relaxes the orbicularis oris, the ring of muscle around your mouth that pulls the upper lip down and in. With a few of those fibers softened, the lip edge turns outward on its own and a bit more of it shows when you smile.

One thing to know going in: the lip flip is an off-label use of Botox. Botox is FDA approved for other uses, and injectors have worked along the lip this way for years, but the lip flip itself is not its own FDA-cleared indication. It comes from the same neurotoxin family we use across our other neurotoxin treatments, whether that is Botox or Dysport.

People also ask whether a lip flip helps a gummy smile, meaning when the top lip lifts high enough to show a lot of gum. Relaxing the lip can soften that look a little, but a true gummy smile is usually treated as its own thing, aimed at the muscles that raise the lip. Whether either one suits you is a conversation, not a guess.
