What is hormone pellet therapy?
Bioidentical hormone pellet therapy replaces the hormones your body has stopped making in the amounts it once did, using a small implant placed just under the skin. At The Spa by Sheena we use the EvexiPEL method for both women and men, and the therapy is run by our medical team rather than sold off a shelf. A single pellet is about the size of a grain of rice. It holds compounded bioidentical hormones, estradiol or testosterone, that are molecularly identical to the ones your own body produces. A provider numbs a small area of the upper outer hip and places the pellet in the fatty tissue just beneath the skin, during a short in-office visit that takes about 15 to 20 minutes. The opening is closed with tape, not stitches, and our providers are certified in the EvexiPEL insertion technique.
Your dose is set from your own lab work, so two people rarely get the same amount. Once a pellet is in, it dissolves slowly and releases hormone as it goes, then it is re-inserted roughly every 3 to 4 months as it wears down. Women generally need a fresh pellet a little more often than men, and your provider sets your own schedule from your labs and how you feel, not a fixed calendar. There is no daily pill to swallow and no gel or cream to apply, which is the part a lot of people are after.
Hormones change as we age. In women, estrogen drops around menopause, which in the United States happens on average in the early fifties, and that decline is what drives hot flashes, night sweats, broken sleep, mood changes, and lower libido. In men, testosterone falls more slowly, by roughly one percent a year after age 30, and genuinely low levels can show up as low energy, low sex drive, changes in mood, and loss of muscle. Pellet therapy is one way to work on the symptoms tied to those shifts, though a provider confirms what is going on with bloodwork before anyone starts.

What to expect from HRT pellet therapy
The insertion itself takes 5 to 10 minutes after numbing. A small bandage stays at the site, which you keep dry for 24 to 48 hours. Mild bruising and tenderness at the insertion site are normal for several days.
Avoid soaking the insertion site (no swimming, baths, or hot tubs) for 5 days. Skip heavy lower-body exercise for 48 to 72 hours to let the site fully seal.
Initial effects from the pellets start within 7 to 14 days and stabilize over 4 to 6 weeks. We re-check labs at the 4 to 6 week mark on the first cycle to confirm dosing is on target, then again before each subsequent insertion.

Everything starts with lab work and a Hormone Therapy Consultation, which is $150. We do not post a pellet price, and that is deliberate. A provider reviews your bloodwork, your symptoms, and your history first, then sets both your dose and your pricing from what the labs actually show. Nothing is prescribed before someone reads your results.
This is medical care, not a retail purchase. Becca (MSN, FNP-C) and Angie (RN) run the program under our medical director, Dr. Arledge, a plastic surgeon, and founder Sheena Klos opened The Spa in 2016. We do not accept or bill insurance, so ask us about cost at your consultation.
Here is something we would rather say plainly. The pellets we use are compounded bioidentical hormones, and compounded products are not FDA-approved. The FDA does not review them for safety, effectiveness, or quality the way it reviews many pills, patches, and gels. We use them under medical supervision, dosed to your labs, and we are not going to tell you they are safer or more natural than FDA-approved hormone therapy. Whether pellets fit you is a medical decision, which is exactly why it starts with a provider.
Bring your full medical history to the consultation. Hormone therapy is not right for everyone. Your provider will ask about things like a history of cancer or blood clots, and, for men, whether you may want to father children, since these change what is safe. Because a pellet cannot easily be removed once it is placed, your provider rechecks your labs and your symptoms over time rather than setting it and forgetting it. Expectations are individual, too. We set them honestly with you instead of promising a result, and high-quality studies on compounded pellets specifically are limited, so we stay careful about what we claim.

What is hormone replacement therapy (HRT)?
Bioidentical hormones are structurally identical to the hormones your body produces naturally. They're derived from plant sources (typically yams or soy) and processed to match human hormones molecule-for-molecule. This is different from synthetic hormones (like Premarin) which have a slightly different molecular structure and different effects on the body.
What are bioidentical hormones?
Bioidentical hormone pellets are rice-sized hormone implants placed under the skin during a quick in-office procedure. They release a consistent dose over 3–5 months for women and 4–6 months for men. Pellets avoid the absorption variability and daily compliance issues of creams and oral medications.
What are hormone pellets?

Does pellet insertion hurt?

We start with a complete hormone panel. HRT isn't appropriate for everyone—we screen for contraindications including history of certain hormone-sensitive cancers, blood clot disorders, and active cardiovascular disease.
Who is a candidate for HRT?
Most patients notice improvement in energy and mood within 2–4 weeks. Sleep and libido often improve in 4–8 weeks. Body composition changes (fat loss, muscle gain with consistent training) develop over 3–6 months. Full optimization is usually realized after 1–2 pellet cycles as we fine-tune dosing.
How quickly will I feel results from HRT?
When properly screened and monitored, yes. We screen for contraindications before starting, monitor lab work regularly, and adjust dosing based on response. Bioidentical hormones at physiologic doses have a strong safety profile in patients without contraindications. We work within evidence-based protocols, not 'megadoses' that can cause issues.
Is HRT safe?
Men receive testosterone pellets, re-inserted every 5 months on average. Women may receive estradiol and/or testosterone pellets, re-inserted every 4 months on average. Both pathways are designed around individual labs and symptoms.
How is HRT for men different from HRT for women?
HRT starts with a $150 consultation. Pellet insertion pricing is set after labs because the right dose depends on your individual hormone levels. We'll go over exact pricing at your consult.
