What is Xeomin?

Xeomin is an FDA-approved wrinkle relaxer made by a German company called Merz. It's in the same family as Botox — same active ingredient, same mechanism, same 3 to 4 month duration, same 1:1 unit dose (a unit of Xeomin is priced and dosed just like a unit of Botox). But there's one important difference.

Xeomin is the "clean" wrinkle relaxer. It's filtered to remove the accessory proteins that come with Botox and Dysport, so what ends up in your face is just the active ingredient — nothing else. Merz built it this way for a reason.

Over time, some clients develop a mild immune response to those extra proteins. When that happens, their Botox stops working as well — the result fades faster, the softening isn't as complete, or a dose that used to be perfect suddenly feels light. Because Xeomin doesn't contain those extra proteins, it sidesteps the resistance problem entirely. In clinical use, Xeomin has a very low rate of antibody formation.

That's why we most often recommend Xeomin to clients who have been getting Botox for years and are starting to feel like it's not working as well as it used to. A switch to Xeomin often restores the full response.

Xeomin has been used clinically for over 15 years and FDA-approved in the U.S. for cosmetic use since 2011.

What to expect from your Xeomin appointment

Xeomin starts working in the same window as Botox — most clients see softening in 3 to 5 days, with full result by day 14. In head-to-head studies, Xeomin actually reached its peak effect faster than Botox (day 4 vs day 8) in some measures, though most clients experience them as similar.

After your appointment, keep it easy for 24 hours: stay upright for 4 hours, no makeup for 3 hours, no exercise or sweat for 12 hours, no flights for 48 hours. Little bumps at the injection sites are normal and gone within the hour. If you bruise, it clears in a week or so.

Duration is 3 to 4 months — same rhythm as Botox. Most clients rebook every 3 to 4 months. If you were coming in for Botox before and switched because it stopped working as well, you'll often find your Xeomin result feels closer to your original Botox response.

Inside a Xeomin appointment

Every Xeomin visit starts with a short consultation. Your injector watches how your face moves, asks about your health history and the medications you take, and maps where each unit goes. The product goes in with a very fine needle across a few small injections, and the appointment usually runs about 15 minutes. Your injector is Becca (MSN, FNP-C) or Angie (RN), both working under our medical director Dr. Arledge, a plastic surgeon. The Spa has been open in Lubbock since 2016.

You will not see a change the day of your appointment. Most people notice movement easing within a few days, with the full effect around two weeks in. It then wears off slowly, and most people come back for maintenance in about three to four months, which lines up with Botox. Xeomin is $14 per unit, billed by the unit rather than by the area, so your total depends on how many units your muscles need. We do not accept or bill insurance, and cosmetic neurotoxin is not covered anyway.

Side effects tend to be mild and short-lived. The most common one is a headache in the first day or two. You may also see minor redness, swelling, or a little bruising where the needle went in. A less common effect is a temporary droop of the eyelid or brow, which happens when a small amount of product settles into a nearby muscle; it fades on its own as the Xeomin wears off. We cannot promise a specific outcome, and results do vary from one person to the next.

Xeomin is not right for everyone. It should be avoided during pregnancy and breastfeeding, and it is not recommended if you have a neuromuscular condition such as myasthenia gravis, ALS, or Lambert-Eaton syndrome, since these can raise the risk of muscle weakness. Tell your injector if you take an aminoglycoside antibiotic, like gentamicin, because it can strengthen the toxin's effect. The best way to find out whether Xeomin fits you is to ask. How Xeomin compares to Botox, Dysport, and Daxxify

Xeomin sits right next to Botox on our menu, and for good reason. Both are botulinum toxin type A, both quiet the muscle so the line above it softens, and both are $14 per unit here. The dosing runs close, too. Trials treating frown lines have used Xeomin and Botox at the same 20-unit dose and found the results hold up the same way, so your injector plans a similar number of units to what Botox would take. We still settle the exact number at your visit, once we see how your face moves.

The real difference is in the formula. Botox and Dysport carry the active toxin bundled together with accessory proteins left over from how the toxin is made. Xeomin is purified down to only the 150 kDa neurotoxin, with those complexing proteins stripped away. That is why people call it the "naked" toxin. Same active molecule, nothing riding along with it.

Some patients choose Xeomin for that reason. The thinking is that fewer added proteins may give the immune system less to react to over years of treatment, since the body can sometimes build antibodies that make a neurotoxin work less well. Studies have reported low rates of those antibodies with Xeomin. We want to be straight with you, though. That is a reason some people prefer it, not a promise that Xeomin prevents resistance or that it will work when another product has stopped. If it is on your mind, raise it at your consultation.
