What is hand filler?

Hand filler restores volume to the backs of the hands, where age, sun, and thinning skin leave the tendons, veins, and bones showing through. As the fat pad under the skin gets thinner, those structures stand out and the hands can read older than the face. We use two different products for the hands, and they are not the same material. One is Voluma, a hyaluronic acid gel from Allergan. The other is Radiesse, made by Merz, which is calcium hydroxylapatite rather than hyaluronic acid. Both add volume, but they behave differently once they are in, and one can be reversed while the other cannot. We explain that below so you can choose with your eyes open. Your injector will talk through which one fits your hands at the consultation.

What to expect from your hand filler appointment

Does it hurt? Less than most people expect. The filler carries lidocaine to numb as it goes in, and your injector can apply a numbing cream first, so the hands are quiet during placement. The product is spread over the back of the hand and massaged into an even layer. Most of the visit is talking and mapping; the injecting itself is quick.

Expect some swelling, and sometimes light bruising, in the first few days. Hands are in constant use, so puffiness can be a little more noticeable here than on the face for a day or two. It usually settles within several days to two weeks. There is no real downtime, though it is smart to skip heavy gripping and hard hand work for the rest of the day.

One risk deserves plain language. Rarely, filler can enter or press on a blood vessel and block blood flow, which is called a vascular occlusion. It is uncommon, but it is the serious one, and caught early it is usually treatable. This is the real reason injector experience matters more than a low price, and it is why Becca and Angie map the hand carefully before anything goes in.

Filler is not right for every moment. Tell your injector if you are pregnant or breastfeeding, since it is not recommended then as a precaution, and hold off if you have an active infection or breakout on the hands or an autoimmune condition that is flaring. The two products reach volume by different routes. Voluma is hyaluronic acid, a sugar your body already makes that pulls in and holds water, so it adds softness and fullness the moment it is placed. Radiesse is calcium hydroxylapatite, tiny mineral microspheres suspended in a gel. It gives volume right away too, but it also prompts your own body to build new collagen over the following weeks, so part of the result comes from tissue you grow yourself.

Radiesse was the first filler the FDA approved specifically for the back of the hands, cleared for that use in 2015 to correct volume loss in the dorsum of the hand. On the hands it tends to last about a year, sometimes longer as the collagen it stimulates matures. Hyaluronic acid fillers like Voluma generally run a little shorter. There is no fixed expiration date with either one; you come back for a touch-up when you notice the volume softening, not on a schedule.

Here is the difference that matters most. Hyaluronic acid is reversible. If you dislike the result, or if it settles unevenly, a provider can inject hyaluronidase, an enzyme that breaks the gel down so your body clears it, and you can read more on our filler dissolving page. Radiesse is not hyaluronic acid, so hyaluronidase does not dissolve it. Once Radiesse is placed it cannot be reversed that way, which is worth weighing before you pick it.
