What is filler dissolving?

Filler dissolving is a short injectable visit that removes hyaluronic acid filler you no longer want. At The Spa by Sheena we use Hylenex, a brand of hyaluronidase. Hyaluronidase is an enzyme that breaks hyaluronic acid down so your body can clear it. It is $350 per vial. Becca (MSN, FNP-C) and Angie (RN) do the injecting here, under our medical director Dr. Arledge, a plastic surgeon, and we have been doing this work in Lubbock since 2016.

People ask to dissolve filler for all sorts of reasons, and none of them is unusual. Some simply do not like how a result turned out. Sometimes filler has migrated, drifting above the lip line where it was never placed. Sometimes there is just too much product, the overfilled look people call pillow face, or a small lump, or one side that sits higher than the other. Under the eyes, where the skin is thin, filler that sits too high can cast a faint bluish shadow called the Tyndall effect. And once in a while it is urgent. If filler presses on or blocks a blood vessel, hyaluronidase is the emergency treatment. One thing decides whether dissolving is even possible. Hyaluronidase only works on hyaluronic acid. That covers the gels we reach for most, including Juvederm, Restylane, RHA, Volbella, and Volux, so the great majority of filler placed on the face can be reversed. It does not work on other materials. Radiesse, the calcium hydroxylapatite we use on the hands, cannot be dissolved this way, and neither can Sculptra, which is poly-L-lactic acid. This is the quiet advantage of hyaluronic acid over permanent fillers. If you change your mind, there is a way back.

What to expect from a filler dissolving appointment

Mild swelling at the treatment area for 24 to 72 hours. Bruising is possible. The tissue may temporarily look slightly hollowed if a large volume of filler is dissolved. This is expected and resolves as the tissue recovers.

Skip strenuous exercise for 24 hours. Avoid pressing on the area. Sleep on your back if the face was treated.

If you plan to re-treat with new filler, wait at least 2 weeks for the tissue to fully recover before re-injecting.

Hyaluronidase works by cutting the bonds that hold hyaluronic acid chains together, breaking the firm gel into small fragments your body absorbs on its own. How many vials you need depends on how much filler is in there and how long it has been settling. A small correction might take only part of a vial. Older filler, or product that has been stacked over several visits, can take more. Your injector sets the amount at your consultation, once she can see and feel what is actually there, so there is no single number that fits every case.

It works quickly. Many people notice a change within a few hours, and most of the hyaluronic acid is broken down within about twenty-four to forty-eight hours. It does not always finish in one sitting. Denser or long-standing filler can need a second visit a couple of weeks later to clear the last of it, and complete settling can take up to two weeks. Needing a touch-up round is common and does not mean anything went wrong.

Expect some swelling afterward. The area can look puffy and feel tender for a few days, from both the injections and the extra fluid, and a little bruising is common. It usually eases within a few days to a week. If you plan to replace the old filler with fresh product, wait about two weeks before re-filling. Hyaluronidase stays active in the tissue for several days, so new filler placed too soon can start to break down before it sets. The pause also lets your injector read your natural shape before adding anything back.

Because hyaluronidase is often derived from animal sources, an allergic reaction is possible, though it is rare. Tell your injector if you have ever had a serious reaction to bee or wasp stings, since that raises the risk, and she may do a small skin test before treating you. An active infection, cold sore, or breakout at the site is a reason to wait until it clears. As with any injectable, an honest health history is part of a safe treatment, and no honest injector will promise you an exact result.
