For the first 48 hours, skip strenuous exercise, saunas, hot yoga, and anything that brings heavy heat or sweating — it can worsen swelling and bruising while the area settles. Keep alcohol light for the same reason. Do drink plenty of water: Sculptra rebuilds volume by prompting your own collagen over months, and staying hydrated supports your skin through that process. A gentle walk is fine.

Trust the timeline
Sculptra rewards patience. Expect a flat phase around week 2, when the swelling is gone but new collagen has not yet built — your face may briefly look untreated, and that is normal. Resume skincare on day 3
You can return to your regular skincare routine on day 3, including active ingredients like retinoids, vitamin C, and gentle acids if you already use them. Here is why that is safe: Sculptra sits deep in the dermis, and your surface skincare does not reach or disturb it. The one caution is to stay gentle over any spot that still feels tender for the first few days — no aggressive scrubbing or hot, vigorous cleansing directly on a bruise. If your skin runs reactive or you are unsure about a specific product, ask us at your follow-up.
Sleep face up (first night)
For the first night, sleep on your back with your head slightly raised if you can manage it. Firm, sustained pressure from side- or stomach-sleeping in those early hours — before you have finished the first day of massage cycles — can nudge freshly placed product unevenly before it settles. Raising your head also helps overnight swelling drain rather than pool in your cheeks and under your eyes. After a night or two, once you are massaging on schedule and the swelling is easing, you can go back to your usual sleep position.
Cold compress (first 24 hours)
For the first 24 hours you can use a cool compress or ice wrapped in a thin cloth on the treated areas — roughly 10 to 15 minutes on, then a break, repeated as needed to ease swelling and settle bruising. Never rest ice directly on bare skin, and keep the pressure light. One exception matters: do not ice right before or during your 5-5-5 massage sessions. The tissue should be warm and pliable when you massage so the product moves freely, so schedule your cold breaks between massages, never during them.
Hydrate aggressively
Drink plenty of water in the days and weeks after treatment — around 8 glasses a day is a reasonable target if that fits your health needs. Sculptra works by prompting your own body to build new collagen slowly over months, and staying well hydrated supports your skin through that rebuild. Good hydration also helps the water-based carrier in the product settle and reabsorb comfortably. It costs nothing and it is one of the easiest things you can do for your result. If a health condition limits your fluids, follow your doctor's guidance instead.
Makeup after 24 hours
Go bare-faced for the first 24 hours. Skipping foundation and heavy makeup gives the tiny injection points time to close cleanly and lowers the chance of pushing bacteria into them. After 24 hours, clean mineral makeup applied with freshly washed hands or a clean brush is fine. If you have a little bruising, light concealer over those spots is usually okay after about 12 hours — just dab it on, do not drag, so you are not working makeup into a site that is still closing. Clean brushes matter more here than which product you pick.
The 5-5-5 massage protocol (critical)
This is the single most important thing you do after Sculptra. Once the numbing has fully worn off — usually 6 to 8 hours after your appointment — massage each treated area for 5 minutes, 5 times a day, for 5 straight days. Use the flat of your fingers or your palm, firm enough to actually move the tissue rather than glide across the surface. That pressure spreads the poly-L-lactic acid evenly so your new collagen builds smoothly instead of clumping into small bumps. Set five phone alarms, and do not skip a day.

Somewhere around week 2, the early swelling has fully resolved but your body has not yet built meaningful new collagen. Bruising (up to 10 days)
Bruising is the most commonly reported reaction after Sculptra, since the product is delivered by injection. Small pinpoint bruises at the injection sites can last anywhere from a few days to about 7 to 10 days. In the cheeks, a bruise may look like it is drifting downward over time — that is simply gravity moving settled blood, not a sign anything is wrong. Some people find arnica gel or oral arnica helps bruises fade, though the evidence is mixed, so check with us before adding anything new. Avoid blood-thinning supplements like fish oil or high-dose vitamin E unless your provider has cleared them.
Swelling and tenderness (first 5 days)
Mild swelling, tenderness, and the feeling of small bumps in the treated area are all expected for the first 2 to 5 days. Here is the part clients most often misread: the fullness you see right after treatment is mostly the water-based carrier the product is mixed in — sterile water, along with mannitol and carboxymethylcellulose — not your result. Your body reabsorbs that fluid within a few days, and the area may look flatter afterward. That is completely normal. The real, collagen-built change arrives gradually over the following weeks and months.

Skipping Massage

Heat & Sun
Avoid saunas, steam rooms, hot tubs, and direct sun exposure for 48 to 72 hours. Heat dilates blood vessels and prolongs swelling and bruising.

Strenuous Exercise
Avoid vigorous exercise, hot yoga, and HIIT for 24 to 48 hours. Elevated heart rate worsens swelling and bruising in the immediate post-treatment period.

Active Skincare
Avoid retinoids, AHAs, BHAs, and vitamin C for 48 hours. After that, resume your normal routine — surface skincare does not interact with deep-dermis Sculptra.

Picking & Bumps
Do not pick at any injection-site bumps or scabs. The product is sitting beneath the skin and the bumps will reabsorb within days. Picking risks scarring.
When to Call The Spa About Your Sculptra

Some swelling, tenderness, bruising, and the feeling of small bumps for the first several days after your Sculptra treatment are all normal and expected. Call us right away if you notice any of the following:
- Skin color changes in or around the treated area — white, grey, blue, or a dark, dusky purple discoloration, especially with pain. This can signal a vascular occlusion and needs to be evaluated within hours, not days.
- Severe or worsening pain out of proportion to the procedure — particularly throbbing or burning that gets worse after the first day rather than easing.
- Any vision changes — blurred vision, double vision, or vision loss. Go to the emergency room immediately and call us on the way.
- Signs of infection — spreading redness, warmth, swelling that worsens after the first 24 to 48 hours, pus, or fever.
- Firm or visible lumps that persist or grow — most early bumps soften with diligent 5-5-5 massage, but any nodule that lasts beyond a few weeks, enlarges, or turns tender, red, or warm should be checked by your injector rather than self-treated.
