What to expect

This guide covers what's normal — and what isn't — for women on bioidentical hormone replacement therapy. It's the companion to your pellet insertion aftercare, focused on the months that follow as your body adjusts and stabilizes on the new hormone levels.

Why this matters

Your body has been operating at a hormone level it adapted to, even if those levels were lower than ideal. As your levels rise on BHRT, your body recalibrates — water balance shifts, mood and energy stabilize differently, hair cycles reset, and the uterine lining may respond. Most of what happens in the first few months is your physiology recalibrating, not a complication. This page helps you tell the difference.

Aftercare restrictions

Following treatment, clients may be advised to avoid strenuous exercise, swimming, hot tubs, or excessive sweating for a short period to allow proper healing. Your provider will give personalized aftercare instructions to help ensure optimal results and comfort throughout the adjustment period.

Post care

Fluid Retention & Swelling

  • Temporary water retention and a 2 to 5 pound weight change are common, especially after your initial insertion.

  • Swelling of the hands and feet may occur, more noticeably in hot, humid weather.

  • To manage: drink plenty of water, reduce salt intake, and take cider vinegar capsules daily.

  • Use a mild diuretic only if prescribed by your provider.

Mood, Energy & Skin

  • Mood swings or irritability may occur if hormone levels were significantly deficient. These typically resolve as levels stabilize.

  • Temporary facial breakouts may occur and are usually short-lived; manage with gentle cleansing, astringents, and toner.

  • Contact your provider at The Spa if breakouts persist.

Hair & Hormonal Tissue

  • Hair thinning is rarely caused by hormone therapy itself and is more commonly linked to prior deficiency.

  • Hair follicles may shed older hair as they transition into a new growth phase. Shedding may last several weeks.

  • Minoxidil (Rogaine), supplements, or topical options from your provider may help support regrowth.

  • Testosterone may also stimulate new hair growth on the chin, chest, nipples, or lower abdomen. Dosage adjustment can reduce, but rarely eliminate, this effect.

Uterine Bleeding & Hormone Regulation

  • Uterine spotting or bleeding may occur in the first few months, particularly if progesterone is not taken as prescribed.

  • This is not necessarily a uterine problem — the uterus may shed existing tissue in response to increased hormone levels.

Downtime

Downtime after Female BHRT is typically minimal, allowing most clients to return to normal daily activities shortly after treatment. Some individuals may experience mild tenderness, swelling, bruising, or redness at the injection or pellet insertion site, which usually resolves within a few days.

  • Vaginal bleeding of any kind — call your provider immediately during business hours.

  • Severe mood changes or symptoms that interfere with daily life.

  • Persistent breakouts or skin changes that do not resolve.

  • Excessive swelling or weight gain beyond the expected range.

  • Any reaction that feels outside the norm.

Follow up

Most adjustment effects resolve as your hormone levels stabilize over the first few months. Bring any persistent changes to your lab review appointment, which is scheduled 5 weeks after your initial pellet insertion. Your provider may adjust your dose based on how your body has responded.
